A 35-year-old couple approached Jindal IVF after facing significant fertility challenges. The husband was diagnosed with obstructive azoospermia, and initial consultations led them to consider Intrauterine Insemination (IUI) with donor sperm. To retrieve sperm, the medical team employed Percutaneous Epididymal Sperm Aspiration (PESA)—a specialized procedure that extracts sperm directly from the epididymis in cases of obstructive azoospermia. The retrieved sperm was then used for Intracytoplasmic Sperm Injection (ICSI) to fertilize eggs collected from the wife’s ovaries. Despite financial constraints that limited the couple’s options for embryo pooling, the process resulted in a single high-quality embryo.
The embryo transfer yielded unexpected results, as the single embryo split after implantation, resulting in a Monochorionic Diamniotic (MCDA) twin pregnancy, where identical twins share a single placenta. While this condition is rare and associated with significant risks such as Twin-to-Twin Transfusion Syndrome (TTTS), the team at Jindal IVF closely monitored the pregnancy, ensuring the best possible outcome.
During the pregnancy, the team identified severe complications requiring critical intervention. Due to the unequal distribution of blood supply between the twins, cord coagulation of one twin’s umbilical cord was performed to preserve the health of the other twin. This intervention allowed the pregnancy to progress successfully to 34 weeks, culminating in the birth of a healthy baby.
